Could M.D. be Mike Esposito? He used the pseudonym Mickey Demeo in the 1960s to hide the fact that he was also working for Marvel while he was on DC's staff.

I know it's late, but I just realized I'd gotten my "D.A." Kirby inkers mixed up...this one is Dick Ayers, not Dan Adkins.
